Should I Use Pepcid or Omeprazole for Heartburn?

Heartburn is one of the most common digestive problems, but choosing the right treatment can be confusing.
Two of the most common over-the-counter options are Pepcid (famotidine) and omeprazole (Prilosec OTC).
While both reduce stomach acid, they work differently and are used for different types of heartburn.
From a pharmacist’s perspective, the best option depends on how often your symptoms happen and how quickly you need relief.
Pepcid (Famotidine): Better for Occasional Heartburn
Pepcid is part of a group of medications called H2 blockers. It works by reducing the amount of acid your stomach produces, and usually begin working within 30–60 minutes, making them helpful for:
Occasional heartburn
Symptoms after spicy or heavy meals
Nighttime heartburn
Preventing symptoms before trigger foods
You can use Pepcid as needed or once to twice daily to control heartburn symptoms.
Omeprazole (Prilosec OTC): Better for Frequent Heartburn
Prilosec OTC belongs to a stronger group of medications called proton pump inhibitors (PPIs). These medications reduce stomach acid more significantly that H2-blockers, making them more effective for:
Frequent heartburn (2 or more days per week)
Ongoing acid reflux symptoms
Recurring symptoms that keep coming back
Omeprazole can be used for severe cases, and it is usually taken daily instead of only when symptoms appear.
The Biggest Difference: Fast Relief vs Long-Term Control
Pepcid works faster and is better for occasional symptoms or meal-related heartburn, while Omeprazole works more slowly, but provides stronger and longer-lasting acid suppression for people with frequent symptoms.
How to Use Pepcid (Famotidine)
Pepcid(Famotidine) is usually taken as needed for occasional heartburn or before meals that commonly trigger symptoms.
It often starts working within 30–60 minutes, making it a good option for faster relief.
For adults take 10–20 mg once or twice daily depending on how long the symptoms persist. You can also take it 30–60 minutes before eating spicy, greasy, or acidic foods to help prevent heartburn before it starts.
How to Use Prilosec OTC (Omeprazole)
Prilosec OTC(Omeprazole) works differently because it is designed for frequent heartburn rather than immediate relief. It is usually taken once daily, ideally 30–60 minutes before breakfast, for a 14-day treatment course.
Unlike Pepcid, omeprazole may take several days to reach full effect, so it should be taken consistently each day rather than only when symptoms appear.
It is commonly used when heartburn happens two or more days per week, or when there is an irritation to the mucosal membranes, active or suspected ulcers.
Similar to Omeprazole
Omeprazole is not the only PPI available over-the-counter, but there are other products that work the same way and can be used interchangeably, these are Nexium(Esmoprazole), and Prevacid(Lansoprazole).
When to See a Doctor
Occasional heartburn is common, but frequent symptoms should not always be self-treated long term.
Seek medical advice if:
Heartburn happens several times every week
Symptoms continue despite medication
