Dextromethorphan is a cough suppressant that works centrally in the brain to reduce the cough reflex. It is mainly used for dry, irritating cough, especially when coughing becomes persistent or disrupts sleep.

You’ll find dextromethorphan in many over-the-counter cough medicines, available in different brands and formulations. One of the most well-known is Delsym 12 Hour Cough Relief, but this raises an important question!
Is Delsym A Regular Dextromethorphan?
Not exactly.
Delsym contains dextromethorphan, but in a different form called dextromethorphan polistirex. This formulation is designed to release the medication slowly over time.
In contrast, most other cough syrups contain immediate-release dextromethorphan, which works faster but doesn’t last as long.
What Makes Delsym Different?
The main difference is how long it works.
Delsym: lasts up to 12 hours
Regular dextromethorphan products: last about 4–6 hours
Because of this, Delsym is taken less frequently, usually every 12 hours, while other products may need to be taken multiple times a day.
This makes Delsym especially useful for overnight relief or when you want consistent symptom control.
Does It Work Better?
Delsym is not necessarily stronger—it simply works longer.
Same active ingredient (dextromethorphan)
Different release mechanism
Fewer doses needed
Both types are effective for dry cough, but the choice depends on your needs.
When to Choose Delsym Over Regular Dextromethorphan
Delsym may be the better option if you have:
A persistent dry cough
Cough that disrupts sleep
The need for long-lasting relief without frequent dosing
When Regular Dextromethorphan May Be Better
Immediate-release Dextrmethorphan products may be more suitable if:
You want faster relief
Your symptoms change during the day
You prefer more flexible dosing
Can I Combine Delsym with Other Cough Suppressants
There are certain products that you may combine with Delsym to help with cough suprression.
Benadryl can be combined at night to suppress night cough, but expect some morning drowsiness.
Cough drops can also be combined and they help mostly to sooth the throat, reduce the itching and irritation that leads to dry cough.
Does Delsym Help with Mucous?
Dextromethorphan is a dry cough suppressant, using it when you have thick mucous can make things worse as it could dry the mucous, and make it harder to expel.
